What to Expect When Working at the Detector Microfabrication Facility (DMF) as a Vendor or Contractor

All vendors and contractors performing work at the Detector Microfabrication Facility (DMF) must complete the following steps prior to beginning work onsite. Please remain in close communication with your DMF point of contact (POC) throughout the process and reach out with any questions.

Before You Arrive

Coordinate with Your DMF Point of Contact

Work scheduling and site access must be coordinated with your assigned DMF POC before arriving onsite.

1. Obtain a SLAC Badge

Visit the SLAC Site Access Portal and complete the required access request form.

Please use the following information when completing the form:

  • Project: B057 DMF Project
  • SLAC Point of Contact: Your assigned DMF POC
  • Reason for Coming to SLAC:
     YOUR PROJECT NAME – YOUR COMPANY NAME
  • Work Location: Building 57 (B057)

After submitting the form and receiving approval, you will receive an email containing your SLAC ID #.

2. Complete Required Training

All vendors and contractors must complete the following training prior to arriving onsite:

  • Course 219: Environment, Safety & Health (ESH) Orientation

*Please save your completion certificate and bring proof of completion when obtaining your badge.

**Additional training may be required depending on your role and scope of work.

  • Access the web training portal here.

3. Submit a Job Safety Analysis (JSA)

Complete and submit the provided Job Safety Analysis (JSA) for review and approval.

Please note:

  • The JSA must be reviewed and approved by a DMF ESH Coordinator before any work may begin.

 


 

Day of Work

Required Identification

A REAL ID-compliant driver’s license or passport is required for site access.

Badge Pickup and Pre-Job Requirements

  • Coordinate with your DMF point of contact to schedule an appointment at the Badge Office in Building 53.
  • Your DMF POC will provide a wet signature on Form A.
  • Your DMF POC will conduct a pre-job briefing with your team.
  • The approved JSA will be reviewed with your team prior to the start of work.
